A person of any age with any type of medical condition or disability may enroll.

Veterans can file for Veterans Affairs (VA) disability compensation, which offers a monthly tax-free payment to Veterans who got sick or injured while serving in the military and to Veterans whose service made an existing condition worse.

AbleGamers’ peer counseling program crafts individualized strategies for people with disabilties to develop solutions for accessibility challenges, combat social isolation and join a worldwide community of players.

The Central Directory of Early Intervention Resources helps parents, family members, service providers, and members of the public find resources for infants and toddlers with developmental disabilities or delays and concerns.

If your wheelchair/mobility device was purchased through Medicare, you may qualify for repair service and cleaning. If your current wheelchair/mobolity device is broken beyond repair or over five years old, they can help you get a replacement.

Adult Day Health Care is available to Veterans for social activities, peer support, and recreation. Can be a half day or full day program. Services may vary by location and may include assistance with transportation to and from a Health Care center.

Pomona CBAS (Community-Based Adult Services) offers a community-based day health program that provides services to adults with chronic medical, cognitive, or behavioral health conditions and/or disabilities. Must be Medi-Cal eligible.

California Department of Public Health offers free referrals for seniors and their families to facilities that offer nursing home services.

America’s Drug Card offers a prescription drug discount card to the general public regardless of age or income. The discount drug card can be used once or more. Discount card provides, on average, a discount of 15% on brand name and 55% on generic drugs. The card covers the entire household and there are no deductibles or claim forms to file.

AbleGamers provides a free online resource that helps educate gamers with disabilities about assistive technologies that remove barriers from making video games accessible.

The Response to Human Trafficking Program (RHTP) provides assistance to human trafficking victims. Services include health care, job training and placement, food, clothing, and shelter. Services are offered by various agencies throughout New York.
